This report has been revised by the assessor further to an internal investigation by HCVN's Management Committee in response to an anonymous complaint of plagiarism (which is a breach of the Licensed Assessor Code of Conduct if proven true). The investigation found that there were similarities in content to another published report, some of which could not be explained by the shared context of the assessment. The evidence presented by the three licensed assessors involved did not however prove beyond reasonable doubt an intention to plagiarise. Hence, the HCVN Management Committee recommended revision of the report to address areas of similarity. The Management Committee considered the revisions made sufficient to close the internal investigation, since this report no longer resembles the earlier report.
